What is "Medication Synchronization"?

Medication Synchronization (MedSync), is a program in which patient medications are individually reviewed by a clinical pharmacist on a monthly basis.

During this review process, the pharmacist will reach out to each patient to verify any medication or diagnosis changes, new or worsening side effects, and patient outcomes of medication therapy.

Following medication reviews, the pharmacist and patient will work together to pick a day of the month when all prescription medications will be filled and ready for pick up!
